Each state issues its own license under its own medical practice act, and you follow that state&#8217;s laws. The compact only routes your application through one shared process. You cannot enter the compact through Hawaii or Vermont, though you can get licenses there. And the four implementation-in-process states cannot issue compact licenses yet, so ask the state board for a go-live date. Only a legislature can add a state, and the map shows no California bill, so a California license still requires a standard application. If you lose it and cannot redesignate (which is free), every compact license you hold is terminated.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Meet the General Requirements<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">You need an accredited medical school degree (or one listed in the International Medical Education Directory), ACGME- or AOA-accredited residency, and passing scores on each USMLE or COMLEX-USA component in no more than three attempts. You also need current or time-unlimited ABMS or AOABOS certification. You cannot have license discipline, a criminal history, controlled-substance actions on your license or an open investigation.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Where Physicians Get Turned Away<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The Canadian LMCC exam does not satisfy the exam rule. Training outside the U.S. counts only if the program held ACGME or AOA accreditation while you attended. Living outside a member state is fine if you meet one of the four SPL tests. Check everything first, because the fee is non-refundable.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What Is an IMLC Letter of Qualification (LOQ)?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">An IMLC Letter of Qualification is the document your SPL issues after verifying your credentials and completing your background check. It confirms you are eligible and lets you request licenses from other member states. It is not a license, and it does not let you practice anywhere by itself.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">What the LOQ Does and Does Not Do<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The LOQ unlocks the step where you choose states and pay each fee. It does not guarantee a license. A board can still ask for a jurisprudence exam, an online profile or proof of work authorization, and ignoring those requests can lead to action against the license.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">How Long Is an LOQ Valid?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">An LOQ is valid for 365 days, with no waivers. During that year, you can add states for a $100 handling fee plus each state&#8217;s license fee. After it expires, you reapply and pay the application fee again, but you don&#8217;t need to show board certification again. List every state you might need in year one.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How to Apply Through the IMLC, Step by Step<\/span><\/h2>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Confirm eligibility using the rules above.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Choose your SPL and start the application on imlcc.com. Some boards are faster.
